29 CFR 1910.305(d)(2):  Panelboards were not mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.    a) Operator's Building (Control Room) - On or about 3/7/13, employees were exposed to electrical hazards when energized electrical panelbox and dead front was missing.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  a) Shop - On or about 3/7/13, employees were exposed to electrical hazards when extension cord was missing ground prong and was not tagged out.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
