29 CFR 1910.213(b)(3):   Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure:  (a) On or about March 26, 2013 the inspection revealed that employees were potentially exposed to serious injury from the use of the Delta Drill Press, serial number 94B70502, without a device to prevent automatic restart. The drill press was located in the NE corner of the East Bay/Engine Bay at the Rigdon/Flatcreek Facility.

29 CFR 1910.181(g)(1):   The employer did not ensure that thorough inspection of all ropes in use was made at least once a month nor that a certification record which included the date of inspection, the signature of the person who performed the inspection, and an identifier for the ropes which were inspected was prepared and kept on file where readily available. The employer did not ensure that any deterioration, resulting in appreciable loss of original strength was be carefully observed and determination made as to whether further use of the rope would constitute a safety hazard:  (a) On or about March 26, 2013 the inspection revealed that employers were potentially exposed to serious injury from the use of a derrick where regular inspections were not performed on the condition of the wire running rope. (a) On or about March 26, 2013 the inspection revealed an outlet designed for use outdoors in a wet environment that had the outer cover broken off. The outlet was located outside in the front of the road crew shop building at the Rigdon Flatcreek facility.
