29 CFR 1926.1403(a): When assembling or disassembling equipment, the employer failed to comply with all applicable manufacturer prohibitions and manufacturer procedures applicable to assembly and disassembly or the requirements of paragraph (b) of this section. a) Super Excavators, Inc. on or about March 27, 2013, at the address of 1501 Black River Pkwy., Lorain, OH: The employer failed to ensure compliance with the manufactures procedures while dismantling the boom on a Kobelco Crawler Crane, Model CK950-11 (serial #GH0201038). Employees performing boom disassembly work on the aforementioned crane were exposed to struck-by/crushed-by hazards after deviating from the manufactures procedures and/or failing to utilize employer developed procedures meeting the requirements under 29 CFR 1926.1406.

Employees performing boom disassembly work from this position and under this condition were exposed to struck-by/crushed-by hazards.

Employees performing this work were exposed to struck-by/crushed-by hazards after connecting pins between the base and ground supported boom tip were removed.
