29 CFR 1926.1403(a): When assembling or disassembling equipment, the employer failed to comply with all applicable manufacturer prohibitions and manufacturer procedures applicable to assembly and disassembly or the requirements of paragraph (b) of this section.       a) Super Excavators, Inc. on or about March 27, 2013, at the address of 1501 Black River Pkwy., Lorain, OH: The employer failed to ensure compliance with the manufactures procedures while dismantling the boom on a Kobelco Crawler Crane, Model CK950-11 (serial #GH0201038). Employees performing boom disassembly work on the aforementioned crane were exposed to struck-by/crushed-by hazards after deviating from the manufactures procedures and/or failing to utilize employer developed procedures meeting the requirements under 29 CFR 1926.1406.

Employees performing boom disassembly work on the aforementioned crane were exposed to struck-by/crushed-by hazards after the employer designated A/D Director failed to evaluate or recognize common hazards associated with this task.

1926.1404(d)(1)(iii): Before commencing assembly/disassembly operations, the A/D director did not ensure that the crew members understand the hazardous positions/locations that they need to avoid.       a) Super Excavators, Inc. on or about March 27, 2013, at the address of 1501 Black River Pkwy., Lorain, OH: The employer failed to ensure that prior to disassembly operations began, the designated A/D Director verified that assigned A/D crews understood the crushing hazards associated with working under the boom. Employees were operating from under the unsupported lattice boom of the Kobelco Crawler Crane, Model CK950-11 (serial #GH0201038) while removing connecting pins. Employees performing boom disassembly work from this position and under this condition were exposed to struck-by/crushed-by hazards.

29 CFR 1926.1405(c): Pins (top or bottom) on boom sections located between the uppermost boom section and the crane/derrick body were removed (partly or completely) when the boom was being supported by the uppermost boom section resting on the ground (or other support).     a) Super Excavators, Inc. on or about March 27, 2013, at the address of 1501 Black River Pkwy., Lorain, OH: During disassembly operations, the employer failed to ensure boom pins on the Kobelco Crawler Crane, Model CK950-11 (serial #GH0201038) were retained in place until appropriate supporting mean were installed/utilized.  Employees performing this work were exposed to struck-by/crushed-by hazards after connecting pins between the base and ground supported boom tip were removed.
