29 CFR 1926.651(k)(2): Where there was evidence of a situation that could result in a possible cave-in or other hazardous condition, the competent person did not remove exposed employees until necessary precautions could be taken to ensure their safety. (a) On or about November 28, 2023, at a job site located near 201 N Rochester St. Rochester, WI, an employee exposed to cave-in hazards while conducting pipe alteration and installation work in an excavation that was not shored, contained significant water accumulation, consisted of type C soil, and was subject to excessive vibration from equipment. The exposed employee was not removed from the cave-in hazard by the competent person or kept from entering the excavation until the shielding system was implemented.

(a) On or about November 28, 2023, at a job site located near 201 N Rochester St. Rochester, WI, an employee worked in an excavation, while involved in activities to include installation, alteration and connection of drainage pipe while working without any means of cave-in protection, exposing the employee to cave-in hazards.
