(a) On or about November 28, 2023, at a job site located near 201 N Rochester St. Rochester, WI,  an employee exposed to cave-in hazards while conducting pipe alteration and  installation work in an excavation that was not shored, contained significant water accumulation, consisted of type C soil, and was subject to excessive vibration from equipment. The exposed employee  was not removed from the cave-in hazard by the  competent person or kept from entering the excavation until the shielding system was implemented.
