29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about April 24, 2013, at the above address jobsite, an employee using a Hitachi 1 ¾ pneumatic coil nailer to secure 30# felt paper was not wearing eye protection equipment while exposed to a potential struck by eye injury.

29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about March 7, 2013, at the above address jobsite, employees performing roof work installing 30# felt paper on a steep pitch roof with unprotected sides and edges approximately 7 ft. 5 inches above a lower level were not protected from falling to that lower level.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   On or about March 7, 2013, at the above address jobsite, employees using a portable ladder set up in an active parking lot to gain access to and from a steep pitch roof approximately 7 ft. 5 inches above a lower level did not have that ladder secured or barricaded to prevent accidental displacement.
