The shear has pneumatic, electrical and kinetic energy. b) On or about May 2, 2013, in the Fabrication Shop, KRRAS Blue Shear, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the shear and/or electrical shock. KRRAS shear, model RAS3013, Serial 1367. The shear has pneumatic, electrical and kinetic energy. c) On or about May 2, 2013, in the Fabrication Shop, Cincinnati Big Gray shear, there were no lockout tagout procedures developed. Employee(s) exposed to the hazard of being caught in the chuck. b) On or about May 2, 2013, in the Maintenance shop, the chuck on the Clark rotating drill press was not guarded. Employee(s) exposed to the hazard of being caught in the chuck. c) On or about May 2, 2013, at the outside slab area, the blade on the Wellsaw pipe cutter was not adjusted to the width of the material being cut. The back guard was missing from the rotating wheel that the blade rotates on. Employee(s) exposed to the hazard from being struck by an unguarded blade. Wellsaw, model 58b, serial number 25275. d) On or about May 2, 2013, in the Fabrication shop area, behind the four shears, there is no guarding to protect employees from being struck by falling pieces of steel being cut by the shear.
