29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     a) On or about May 2, 2013, in the Fabrication shop, Cincinnati Big Green Shear, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the shear and/or electrical shock.  Cincinnati shear, number #41620, 2CC12. The shear has pneumatic, electrical and kinetic energy.    b) On or about May 2, 2013, in the Fabrication Shop, KRRAS Blue Shear, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the shear and/or electrical shock. KRRAS shear, model RAS3013, Serial 1367. The shear has pneumatic, electrical and kinetic energy.     c) On or about May 2, 2013, in the Fabrication Shop, Cincinnati Big Gray shear, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the shear and/or electrical shock. Cincinnati big gray shear, serial number 21K01. The shear has pneumatic, electrical and kinetic energy.     d) On or about May 2, 2013, in the Fabrication Shop, Cincinnati near the plasma cutter, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the shear and/or electrical shock. Cincinnati green shear, near the plasma cutter, model 1810, serial 34656. The shear has pneumatic, electrical and kinetic energy.     e) On or about May 2, 2013, in the Fabrication Shop, Cincinnati #1 press brake, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in the press and/or electrical shock. Cincinnati 135 ton press brake #1, near the entrance door, serial number 39881. The press has pneumatic, electrical and kinetic energy.     f) On or about May 2, 2013, in the Fabrication Shop, Cincinnati 225 ton press brake, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in press and/or electrical shock. Cincinnati 225 ton, press brake, serial number 35317. The press has pneumatic, electrical and kinetic energy.     g) On or about May 2, 2013, in the Fabrication shop, Cincinnati 225 ton Big Gray, there were no lockout tagout procedures developed. The press has pneumatic, electrical and kinetic energy.    h) On or about May 2, 2013, in the Fabrication shop, Chicago 135 ton press, there were no lockout tagout procedures developed. The punch press has electric and kinetic energy.    j) On or about May 2, 2013, BDI area, Clearing punch press, there were no lockout tagout procedures developed. Employee(s) were exposed to being caught in punch press and/or electrical shock. Clearing, 75 ton, punch press, serial number 51-19270-P. The punch press has electric and kinetic energy.     k) On or about May 2, 2013, the BDI area, Niagara punch press, there were no lockout tagout procedures developed. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a) On or about May 2, 2013, in the Maintenance shop, the chuck on the Clausing Atlas rotating drill press was not guarded. Employee(s) exposed to the hazard of being caught in the chuck.      b) On or about May 2, 2013, in the Maintenance shop, the chuck on the Clark rotating drill press was not guarded. Employee(s) exposed to the hazard of being caught in the chuck.     c) On or about May 2, 2013, at the outside slab area, the blade on the Wellsaw pipe cutter was not adjusted to the width of the material being cut.  The back guard was missing from the rotating wheel that the blade rotates on. Employee(s) exposed to the hazard from being struck by an  unguarded blade. Wellsaw, model 58b, serial number 25275.      d) On or about May 2, 2013, in the Fabrication shop area, behind the four shears, there is no guarding to protect employees from being struck by falling pieces of steel being cut by the shear. Shears not guarded behind are: Big Green Cincinnati shear, model number 41620, the KRRAS shear model 1367, Cincinnati shear, serial number 21K01, and the new green Cincinnati shear model 1810, serial number 34656.

ALNC Inc., was previously cited for a violation of this Occupation Safety and Health Standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in inspection number 311313803, citation number 01, item number 01, which became final order on August 22, 2008, with respect to the workplace located at 2152, FM 2105, San Angelo, Texas.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to install guarding on all the press brakes.
