On or about February 4, 2019 and at times prior thereto, the employer allowed Supervisors to train Plasma Operators to stand on the metal cutting table of CNC Plasma Cutting Machines. Plasma Operator(s) were observed standing on top of CNC Plasma Cutting Machine, ESAB (PL-1), Serial Number 015-102-0261, applying weight onto steel metal sheet by placing feet on the edge of metal, while the machine was in operation, exposing operator(s) to the hazards of being struck by the machine torch.

Plasma Operator(s) were observed standing on top of CNC Plasma Cutting Machine ESAB (PL-1), Serial Number 015-102-0261, observing the cutting of the plasma torch. Operator(s) were wearing Z87 Nemesis Safety Glasses that do not provide proper shading for plasma cutting operations, exposing operators to the hazards of ultraviolet radiation.
