29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a)  Hinkle Manufacturing, Inc., Perrysburg, Ohio:  On or about April 24, 2012, the employer did not ensure that employees were protected from point of operation hazards while cutting product on the #133 Moak manufactured band saw, exposing them to laceration and amputation injuries.  On April 24, 2013, an employee received a laceration injury when their hand inadvertently contacted the saw blade while cutting a piece of foam.    b)  Hinkle Manufacturing, Inc., Perrysburg, Ohio:  On or about May 8, 2013, the employer did not ensure that employees were protected from point of operation hazards while cutting product on the #131 Moak manufactured band saw, exposing them to laceration and amputation injuries.  An employee was observed cutting foam pieces with his fingers 1-2 inches from the cutting blade.
