29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    a) First Floor Rear Stairway - A laborer was observed pushing a wheel barrow filled with brick mortar across a 54 inch wide stairway landing and the floor was cluttered with loose brick, loose block, wooden boards and slippery wet brick dust, thus exposing the laborer to slip and fall hazard, on or about 05/08/13.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:     a) Second Floor North Wall - A mason was observed applying Brick-Loc Type S mortar which contained Portland Cement, during a brick laying operation and was not wearing eye protection, on or about 05/08/13.     b) Second Floor North Wall - A foreman was observed applying Brick-Loc Type S mortar which contained Portland Cement, during a brick laying operation and was not wearing gloves, on or about 05/08/13.
