29 CFR 1926.501(b)(6): Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems: a) Exterior Rear of Building - A laborer was observed walking across a 19 inch wide ramp to gain access to the first floor to retrieve steel rebar and was exposed to a 6 ft, 4 inch fall hazard, on or about 12/04/2013.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: a) Exterior Rear - A mason was observed applying Brick-Loc, Type S brick mortar, which contained Portland Cement, on top of a block wall and not wearing hand protection, on or about 12/04/2013. Traditional Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.95(a), which was contained in OSHA inspection number 905795, citation number 1, item number 2, and was affirmed as a final order on 07/03/2013, with respect to a workplace located at 1508 14th Street, NW Washington DC 20005.
