29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: A the roof level above the smoke glass awning main entrance on May 13, 2013, an employee was using an extension cord with a missing ground pin to provide temporary electrical service to a Black & Decker DR560 1/2-Inch 7.0 Amp Drill/Driver. The Drill/Driver was used in conjunction with a paddle steel mixer to mix paint.
