(a) Mechanical Room, North Warehouse, Defense Commissary Agency, 2170 Arizona Street, Bldg 61610, Fort Huachuca, AZ 85613:  On or about 14 May 2013, a flexible cord was run through a doorway to the Mechanical Room to charge the battery for a Mechanical Floor Scrubber stored in that room, exposing employees to hazards.
