Foster Excavating Inc. worksite located at Whitehouse Valley Phase 3, Whitehouse, Ohio: On or about May 21, 2013, the employer failed to ensure a safe means of egress such as a ladder was provided to employees installing sanitary sewer lines in a trench that was approximately 7.10 feet deep.    Foster Excavating Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651, which was contained in OSHA inspection number 451413, citation number 1, item number 1 and was affirmed as a final order on June 28, 2012, with respect to a workplace located at Perrysburg, Ohio.
