Foster Excavating Inc. on or about May 20, 2020, at the site located near the address of 30600 Drouilliard Rd., Walbridge, OH: The employer failed to ensure employees were adequately protected from cave-ins while an employee was performing work tasks such as installing a catch drain in approximately  7 ft. deep unprotected excavation.
