29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated;  the person certifying that the evaluation has been performed;  the date(s) of the hazard assessment;  and which identifies the document as a certification of hazard assessment:       a) Alloy Cast Products, Inc., Kenilworth, NJ site - Production Area: The Employer did not have a written certification of the hazard assessment.  The Employer did not have each affected employee wear appropriate PPE such as, but not limited to heat resistant sleeves, jackets, spats, etc. to protect them from burn hazards while casting metals such as cobalt alloy, etc.       Condition noted on/or about June 12, 2013.

29 CFR 1910.303(g)(1)(ii): Working space about electric equipment rated 600 volts, nominal, or less was used for storage:            a)  Alloy Cast Products, Inc., Kenilworth,  NJ site - Wax Room: Electrical panel board rated less than 600 volts, nominal or less (240 AC) was not maintained free of storage, in that, storage materials such as, but not limited to 32 gallon (approx.) plastic containers containing Kaolin Clay, Ground Silica, a stool, and other storage containers, etc. obstructed the access way.         Condition noted on/or about May 30, 2013.
