Visual inspection daily; monthly inspection with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected.    1) 700 Swenson Drive Kenilworth  On or about July 17, 2013,  Inspection and certification records were not maintained as required for the Stancion Trolley and Hoist systems including but not  limited to the chains and hooks,  used to lift and pour molten metal.
