29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    On or about June 6, 2013, at the above addressed jobsite, the portable ladder used to access the second floor did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.     There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about June 6, 2013, at the above addressed jobsite, employees performing carpentry operations on the second floor were exposed to a fall of thirteen (13') feet created by the presence of unprotected sides.       The Horizon Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 312727472, citation number 1, item number 1 and was affirmed as a final order on August 6, 2009, with respect to a workplace located at 9957-9967 Cummings Street Huntley, Illinois.     There is no abatement certification or documentation required for this item.
