Rise height and tread width shall be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs.    a.  Located in the facility, there were stairs that employees walked up and down that went from the employee break room to the employee locker room that did not have a non-slip nosing on each of the stair treads, thereby exposing the employees to a fall hazard from slipping on the stairs.    b.  Located at the main line, there were fixed stairs that allowed for access to the top of the line that had significantly bent treads that did not allow for the rise height to be uniform, thereby exposing the employees to a fall hazard from the stairs.
