29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction.  While the cover was not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings:    a)  Boiler Room - On or about June 21, 2013, employees, working where greywater drained into the cistern, were exposed to a round unguarded floor hole 13 inches in diameter, an oval unguarded floor hole of 13 inches wide by 18 inches long, and another round unguarded floor hole 12 inches in diameter.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.23(b)(4): Temporary wall openings were not adequately guarded against falling, but the guards need not be of standard construction:    a)  Laundry Washing Room - On or about June 21, 2013, employees, working with the double doors open for ventilation, were exposed to an unguarded temporary wall opening that created a fall hazard of 27.5 inches from the open doors to the floor.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.23(d)(1): Flights of stairs having four or more risers were not equipped with a standard railing as specified in paragraphs (d)(1)(i) through (v) of this section:    a)  Paint Room - On or about June 21, 2013, employees, accessing the four-riser stairway without a guardrail system, were exposed to a fall hazard of 33 inches from the top landing to the floor.       Abatement certification required within 10 days after abatement date. 29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:  a)  Boiler Room - On or about June 21, 2013, employees were exposed to an incomplete guardrail system while accessing the Boiler Room because the midrail, removed to install the chiller in April, had not been replaced, exposing employees to a fall hazard of 40 inches from the landing onto the water treatment chemicals and then the floor.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
