29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On July 2, 2013, in the rear of the house, employees installing framing while constructing a house were not wearing hard hats while working below employees working on scaffold, exposing employees to the hazard of being struck by falling debris.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:   On July 2, 2013, on the northeast side rear of the house, employees installing framing material were using a 2 inch by 8 inch wood board as scaffold and were not protected from a 6 feet 6 inch fall by the use of a fall protection system.
