29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  (a) 501 Sumner Ave Springfield, MA: On or about July 16, 2013, several employees were observed climbing up nine 5 gallon buckets of roofing adhesive set up in a stair like fashion to reach the upper roof level that was 6+ FT above the lower surface, which is not in accordance with this subpart.
