29 CFR 1910.332(b)(1):   Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:   A)  Ridg-U-Rak, Inc., 12340 Gay Road, North East, PA  16428, on or about July 24, 2013 and times prior thereto:  An employee was not provided with specific training on the location, purpose, and function of the 480V breaker panel box utilized to de-energize and lockout the electric bus bar for the overhead hoist trolley system.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed:   A)  Ridg-U-Rak, Inc., 12340 Gay Road, North East, PA  16428, on or about July 22, 2013 and times prior thereto:  Circuit breaker lockout devices were not provided to employees for locking-out the circuit breakers located in the 220V and 480V electrical panel boxes.   Employees performed electrical servicing and maintenance work on equipment, lighting, receptacles, and the overhead hoist system that were served by the 220V and 480V electrical panel boxes.
