29 CFR 1910.24(b): "Where fixed stairs are required." Fixed stairs shall be provided for access from one structure level to another where operations necessitate regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations. Fixed stairs shall also be provided where access to elevations is daily or at each shift for such purposes as gauging, inspection, regular maintenance, etc., where such work may expose employees to acids, caustics, gases, or other harmful substances, or for which purposes the carrying of tools or equipment by hand is normally required.  a.  On or about July 29, 2013, in the facility, employees were exposed to fall hazards when accessing elevated parts associated with the Base Injection Machine.  Employees were required to access an elevated platform of the machine on a regular basis during color changes and were not provided with fixed stairs.   b.       On or about July 29, 2013, on the platform above the Rikutec Blowmold machine, employees were exposed to fall hazards when accessing the material hopper for the extruder.  Employees were required to access the hopper via a stepladder which elevated the employees above existing guardrails.  Employees were not provided fixed stairs with appropriate railings and handrails.
