29 CFR 1910.179(g)(2)(i): Electrical equipment shall be so located or enclosed that live parts will not be exposed to accidental contact under normal operating conditions. (a) On or about August 20, 2013: In the cab of Gantry Crane #23, the heater fan has missing electrial cover over the control-switch. The operator was exposed to electricution and shock hazards. (b) On or about August 20, 2013: In the cab of Gantry Crane #23, there was no light bulb in the bulb-receptacle. The operator was exposed to an electrocution hazard.

29 CFR 1910.179(l)(3)(i): Any unsafe conditions disclosed by the inspection requirements of paragraph (j) of this section shall be corrected before operation of the crane is resumed. Adjustments and repairs shall be done only by designated personnel. (a) On or about August 21, 2013, on Gantry Crane #76, in the mechanical room, hydraulic fluild was observed on and around the "front drum" as noted on the operator pre-operational checklist. Even though the fluid was leaking, the operator operated the crane on this day. Employees were exposed to crushing and struck-by hazards.

29 CFR 1915.55(e)(1): Fuel gas and oxygen manifolds shall bear the name of the substance they contain in letters at least one (1) inch high which shall be either painted on the manifold or on a sign permanently attached to it. (a) On or about August 20, 2013, near Area 42 at Jeff Boat Ship Building Yard, the employer did not have the name of the gaseous substance painted on the mainfold or a permanent sign attached. This potentially exposed employees to an explosion and or inhaltion hazards.
