29 CFR 1926.652(a)(1):     Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):     On August 22, 2013, an employee was not protected from a cave-in while working in an unprotected trench, exposing the employee to the hazards associated with working in a 13 foot 6 inch deep trench.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
