If such hazards are present, or likely to be present, the employer shall: Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment; a. Finishing Department- On or about October 21, 2013, employer had not conducted a workplace hazard assessment to determine hazards that necessitate the use of personal protection equipment. Employer failed to select proper hand protection to be used when operating the rotary swaging machine. Thereby exposing employees to cuts, lacerations, and amputation(s).

Finishing Department- On or about October 21, 2013, Employees failed to apply proper controls by not properly locking and tagging out a rotary swaging machine while dies were being changed. Employees who perform these tasks were using stop buttons to perform lock-out operations. Thereby exposing employees to unexpected energization of equipment.

Finishing Department- On or about October 21, 2013, Rotary swaging machine did not have legible or distinguishable start or stop button(s) installed on machinery. Employees performing tasks, could not determine if the industrial swaging machine was either turned on or turned off before beginning assigned task(s). Thereby exposing employees to amputation hazard(s).
