29 CFR 1915.153(a)(1): The employer did not ensure that employees used appropriate eye or face protection where there were exposures to eye or face hazards caused by flying particles, molten metal, liquid chemicals, acid, caustic liquids, gases, vapors, or potentially injurious light radiation: On or about October 23, 2013, on-board the USS Holland, 01 level, bow section, an employee was cutting sections of the ship and was not using a face shield for protection because it was damaged.
