29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Lower Level Welding Area - Employees used a metal junction box with missing knockouts to plug in the Miller Renegade 180 welder. The box was not mounted securely to a surface in accordance with the product listing and labeling. Employees were exposed to electrical shock electrocution hazards on or about November 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) Lower Level Fabrication Area - Employees used the Edwards Iron Worker, Serial Number 0914011A to fabricate steel pipe components for agricultural gates and fencing. Employees were exposed to the hazard of electrical shock or electrocution when they turned the machine on and off using a switch with no cover on or about November 14, 2013. (b) Lower Level Fabrication Area - Employees turned the lights on and off at the breaker box. Several knockouts were missing from the breaker panel. Employees were exposed to electrical shock from the live 220 VAC bus in the panel on or about November 14, 2013. (c) Lower Level Fabrication Area - Employees plugged the Miller Renegade 180 welder into a 4 gang junction box with a missing knockout on the back. Employees were exposed to electrical shock from the live 220 VAC wires inside the box on or about November 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) Lower Level Welding Area - Employees used the small Lincoln welder to fabricate galvanized steel farm gates and fencing. The ground pin was missing from the welder's 115 VAC power cord and employees were exposed to electrical shock hazards on or about November 14, 2013. (b) Lower Level Welding Area - Employees used the Air master 3 foot diameter box fan to move air through the shop during welding operations on galzanized steel farm gates and fencing. The ground pin was missing from the box fan's 115 VAC power cord and employees were exposed to the electrical shock hazards on or about November 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Employees used the branch circuit to power the Miller Renegade 180 Welder on or about November 14, 2013. (b) Lower Level Welding Area - The 220 VAC single phase branch circuit cable entered the left side of the circuit breaker panel box through and opening without a connector. Employees used the branch circuit to power the Lincoln 216 power mig welder on or about November 14, 2013. (a) Lower Level Welding Area - The 220 VAC single phase branch circuit cable entered the metal four gang junction box through and opening without a connector. Employees used the branch circuit to power the Miller Renegade 180 Welder on or about November 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Employees used the breakers to turn lighting on and and off and power for the welders passed throug h220 VAC breakers in the box on or about November 14, 2013.
