29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   (a) Lower Level Fabrication Area - Employees used the Edwards Iron Worker, Serial Number 0914011A to fabricate steel pipe components for agricultural gates and fencing.  Employees were exposed to the hazard of electrical shock or electrocution when they turned the machine on and off using a switch with no cover on or about November 14, 2013.    (b) Lower Level Fabrication Area - Employees turned the lights on and off at the breaker box.  Several knockouts were missing from the breaker panel.  Employees were exposed to electrical shock from the live 220 VAC bus in the panel on or about November 14, 2013.   (c) Lower Level Fabrication Area - Employees plugged the Miller Renegade 180 welder into a 4 gang junction box with a missing knockout on the back.  Employees were exposed to electrical shock from the live 220 VAC wires inside the box on or about November 14, 2013.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
