29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) PARKING LOT - Employees installed an eight (8) inch water supply line into an approximate six (6) foot deep trench, and the means of entering and exiting the trench was located approximately forty (40) feet from the employee(s), thereby exposing employees to serious injuries, on or about November 15, 2013. No abatement certification or documentation required.

No abatement certification or documentation required.
