29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about November 13, 2103, employees working on overhead signs were not wearing protective hard hats and exposed to possible danger of head injuries from flying or falling objects when working at lower levels and from impact when moving in and around the large signs.

29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about November 13, 2013, employees working on a sign from an Elliot L-60 hi-reach aerial work platform were not wearing personal fall protection and exposed to the hazard of falling approximately 18 feet to the concrete below.    The Sign Remedy Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v) which was contained in OSHA inspection number 748681, citation number 01, item number 001 and was affirmed as a final order on date01/22/13, with respect to a workplace located at  907 U.S. Highway 67 Duncanville, Texas.
