29 CFR 1926.453(b)(2)(v): 29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.   On the west side of the commercial building, an employee working on a sign from an Elliot L-60 hi-reach aerial work platform was not wearing personal fall protection and exposed to an unprotected hazard of falling approximately 20 feet to the concrete below.   Sign Remedy, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 949071, citation number 02, item number 001 and was affirmed as a final order on 01/03/2014, with respect to a workplace located at 3916 Kemp Blvd, Wichita Falls, Texas 76308.
