29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    On or about December 4, 2013, conveyor 5B was operated with an unguarded shaft, exposing employees to the hazard of being caught in rotating parts.

29 CFR 1910.272(g)(1)(iii): The atmosphere within a bin, silo, or tank in the grain handling facility was not tested for the presence of combustible gases, vapors, and toxic agents prior to employee entry:  On or about December 4, 2013, the employer did not test for the presence of combustible gases, vapors, toxic agents and sufficient oxygen content, exposing employees to the hazard of oxygen deficient and/or hazardous atmosphere.  The Lone Star Grain Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.272(g)(1)(iii) which was contained in OSHA inspection number 903884, citation number 1, item number 5a and was affirmed as a final order on 08/1/2013 with a final abatement date of 08/26/13, with respect to a workplace located at 8778 North FM 1644, Calvert, TX 77837.
