While the cover was not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: a. On or about December 23, 2013, in the test pit area, employees performing pump testing operations were exposed to the hazard of accidentally walking into a pit with an unguarded floor opening measuring approximately 14 inches by 38 inches . The pit was not guarded by either a standard railing or floor opening cover of standard strength and construction.

The proper care, maintenance, useful life and disposal of PPE.
