On or about July 28, 2025, in the production area, employees were required to operate the South Bend Precision Lathe on a daily basis. The employer failed to provide adequate guarding for the South Bend Precision Lathe. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.  a. On or about July 28, 2025, in the production area, employees were required to operate the stationary belt powered grinder on a daily basis. The employer failed to provide adequate guarding for the unguarded pulley drive. Employees were thereby exposed to the hazards associated with unguarded points of operation during the operating cycles.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  On or about July 28, 2025, the employer failed to ensure that employees were protected from a struck by and or caught by hazard when using the bench grinder to deburr metals. The grinder was adjustable tongue guards, was not adjusted withing a one-quarter (1/4") inch, thereby exposing employees to caught in between hazards.
