29 CFR 1910.23(c)(1):      Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings [or equivalent as specified in 29 CFR 1910.23(c)(3)(i) through (v)], on all open sides:       On or about January 15, 2014 employees were exposed to fall hazards of approximately 15ft, while opening and closing lids from the tops of railcars, because the employer did not ensure the work area was guarded by a standard railing or equivalent method.
