29 CFR 1910.134(c)(2)(i):  The employer determined that any voluntary respirator was permissible, the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"):  a) Worksite; on or about February 12, 2014 - Employees were permitted to voluntary use particulate N95 tight fitting respirators during hand air sanding.  Employees were not provided Appendix D of the respiratory standard.

29 CFR 1910.1200(f)(5)(ii):The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) Art/Graphics department; on or about January 21, 2014 - A safety dispenser bottle of Heptane, which contained solvent naphtha was not labeled with the contents on the container.   b) Art/Graphic department; on or about January 21, 2104 - A spray bottle of Application Fluid, which contained propylene glycol methyl ether was not labeled with contents on the containers.
