29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.    On or about February 4, 2013:    a) The employer failed to ensure that all employees received from Pedro Sanchez were properly trained in fall protection, before exposing them to fall hazards of approximately 14 feet while installing dense glass on the facing of a newly constructed building.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about February 4, 2014:    a) Employees were exposed to a fall hazard of approximately 14 feet while working on a flat roof area installing dense glass material where the employer failed to ensure that they were properlly protected from the fall hazard by the means allowed within the standard.    Simco Interiors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 314643073, citation number 1, item number 2 and was affirmed as a final order on 01/04/2012, with respect to a workplace located at 4290 Bells Ferry Rd NW. Kennesaw GA 30144.
