29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Proctor and Gamble Distribution Warehouse, Shippensburg Pennsylvania - Employees dumped snow directly from the south east side of the roof of the distribution warehouse approximately 44 feet above the ground.  The employees were not protected from falling with a guard rail, safety net or personal fall arrest system, on or about February 25, 2014.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:  (a) Proctor and Gamble Distribution Warehouse, Shippensburg Pennsylvania - Employees worked to install roofing material on the decking of the new building.  The warning line was not maintained around the all sides of the work area.  Approximately 50 feet of warning line was laying on the decking along the eastern leading edge, employees were exposed to a fall of 44 feet from the eve to the ground on or about February 25, 2014.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:  (a) Proctor and Gamble Distribution Warehouse, Shippensburg Pennsylvania -  Employees accessed the roof from an elevated scissor lift.  The employer did not establish an access path between the scissor lift and the work area formed by two warning lines.  Employees were exposed to a fall of approximately 44 feet from the roof to the ground below on or about February 25, 2014.  No abatement certification or documentation required.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    (a) Proctor and Gamble Distribution Warehouse, Shippensburg Pennsylvania - Employee(s) worked to install the roofing material on the east side of the distribution warehouse roof.  An exposed employee worked outside of the warning line without a personal fall arrest system, safety monitor, guard rail or safety net and was exposed to a fall of approximately 44 feet from the roof to the ground on or about February 25, 2014.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.    Donald B. Smith Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 960031, citation number 1, item number 1 and was affirmed as a final order on August 21, 2012, with respect to a workplace located at 1305 Roosevelt Ave, York Pennsylvania.
