29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:   On or about February 27, 2014, and at times prior thereto, employees were working on a rig site, Choctaw Rig #11. There were two areas on the platform that lacked railings. One area was where the operator was standing while controlling the rig. Employees were exposed to potential fall hazards of 4 feet or more.

29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side:  On or about February 27, 2014, and at times prior thereto, employees were working on a rig site, Choctaw Rig #11. The stairway to access the rig platform was missing the right rail. This condition exposed employees to fall hazards.
