29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.    On or about February 26, 2014, and at times prior thereto the walkway between the shaker pit and  the trip tank was not flush to the floor. This condition exposed the employees to a trip and fall hazard.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    On or about February 26, 2014, and at times prior thereto, an electrical cable located with the low hanging wires between the shaker pit and the trip tank had insulation damage. This condition exposed the employees to an electrical shock hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:     Drill site: On or about February 26, 2014, and at times prior thereto, the strain reliefs from an electrical switch and a circuit breaker box were damaged. This condition exposed the employees to electrical hazards.   a. Mixing pump switch #5 located at the base of the staircase to mudd pit had a broken strain relief.    b. A cable going into high voltage electrical circuit breaker box located next to the mudd pump #2 had a broken strain relief.

29 CFR 1910.305(a)(2)(xi): Cable assemblies and flexible cords and cables were not supported in place at intervals that ensure that they will be protected from physical damage:    Drill site: on or about February 26, 2014, and at times prior thereto, the 480 volt overhead wires running from the shaker pit to the trip tank were left hanging low. This condition exposed the electrical conductors to potential physical damage.
