29 CFR 1910.134(h)(3)(iii): Air and oxygen cylinders of self-contained breathing apparatus were not recharged when the pressure fell to 90% of the manufacturer's recommended pressure level: On or about March 13, 2014, at the Sidewinder Rig #223, the employer did not ensure the escape cylinders stored in the Driller's cabin were recharged when the pressure fell below 90%. This condition exposes employees to health hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about March 13, 2014, at the Sidewinder Rig #223, the employer did not ensure a portable heater (Model HS175KT) used to heat the employee change room had a guard installed to cover the fan blades. This condition exposes employees to rotating parts and point of operation struck-by hazards.
