29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).     (a) An employee was observed working at the unguarded edge of the barge, removing a flush manhole cover, while exposed to falls of approx. 9 to 10 feet to the water.

(a) An employee was observed working at the unguarded edge of the barge, removing a flush manhole cover, while exposed to falls of approx. 9 to 10 feet to the stairway platform below or into the water.

(a) An employee was observed detaching a large metal piece from the lifting device of the crane and was exposed to head injuries. No hard hat or head protection was worn during the time the crane was making and landing the pick.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws.    (a) The plug end of the orange extension cord with a black stripe was located at the work site and was not connected to prevent strain from being exerted on the conductor's connecting points. No live parts were exposed.
