29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturer's prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.   a) On or about October 7, 2022 and at times prior thereto, employees were exposed to injury by tip or rollover while operating a powered industrial truck with an front-end  attachment other than the factory installed equipment. The approximate weight of the truck in combination with the attachment, at maximum elevation with a load laterally centered, was not identified. This hazardous condition exposed employees to a fall while working within the man basket. 29 CFR  1915.159(c)(6):Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and shall not be used again for employee protection until inspected and determined by a qualified person to be undamaged and suitable for reuse.  (a) On or about October 7, 2022 and at times prior thereto, employees were exposed to a fall in that the 6' fall arrest lanyard had been exposed to an impact and would likely not adequately arrest a fall.
