29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  (a)  Washington Crane: On or about April 3, 2014 and times prior thereto, the employer did not provide swing gates at the top of the ladderway.  This exposed employees to potential falls of up to 40 feet.  (b)  Clyde Crane:  On or about April 3, 2014 and times prior thereto, the employer did not provide a swing gate at the top of the ladderway.  This exposed employees to a potential fall of over 30 feet.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:  Washington Crane: On or about April 3, 2014 and times prior thereto, the railing height on the platform at the top of the ladderway measured at 27 inches on the top rail.  This exposed employees to a potential fall hazard of 40 feet.

29 CFR 1915.56(b)(2): Welding cable was not free from repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected.   (a)  On or about April 3, 2014 and times prior thereto, the employer made repairs to welding cables within ten feet of the electrode holder.  This exposed employees to potential shock hazards from using the cable close to their body.
