The hazard was located in the machine house of the old Washington crane, where employees were potentially exposed to battery acid when servicing batteries.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): (a) On or about April 3, 2014 the inspection revealed that employees were potentially exposed to serious injury from electrical shock because the resistor in the small Washington mechanical room was not guarded against accidental contact by an appropriate enclosure. The resistor was located on the ceiling above the wire rope drum. (b) On or about April 3, 2014 the inspection revealed that employees were potentially exposed to serious injury from electrical shock because the resistor in the Clyde Crane #2 electrical room was not guarded against accidental contact by an appropriate enclosure.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about April 3, 2014 the inspection revealed that employees were potentially exposed to serious injury from the unguarded ingoing nip points and rotating parts on the whipline/auxiliary hoist drum, the main hoist drum and the boom hoist drum on the side located nearest the access door to the crane operator's cab in the Clyde crane #2 at the 80' level machine house. (b) On or about April 3, 2014 the inspection revealed that employees were potentially exposed to serious injury from the unguarded ingoing nip points and rotating parts on the whipline/auxiliary hoist brake, main hoist brake and boom hoist motor brake on the side located nearest the access door to the crane operator's cab in the Clyde crane #2 at the 80' level machine house. (c) On or about April 3, 2014 the inspection revealed that employees were potentially exposed to serious injury from the unguarded nip points and rotating parts on the auxiliary hoist drum, main hoist drum and boom hoist drum located in the machine house of the "Old" Washington Crane.
