29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:  a.) Facility - On or about 5/28/14, a machine operator working on and around Davenport Machines was exposed to continuous noise levels at 90.4% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximlatey 91.3 dBA during the 358 minute sampling period; exposure calulations inlcude a zero increment for the 57 minutes for breaks.  The employer had not instituted a training program in accordance with the requirements of 29 CFR 1910.95(k) and ensured that employees participated in the training.  b.) Facility - On or about 5/28/14, a machine operator working on and around Davenport Machines was exposed to continuous noise levels at 70% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.5 dBA during the 359 minute sampling period ; exposure calculations include a zero increment for the 57 minutes for breaks.  The employer had not instituted a training program in accordance with the requirements of 29 CFR 1910.95(k) and ensured that employees participated in the training.  c.) Facility - On or about 5/28/14, an area sample representing employees' working on the south end of the building consistently at operational Davenport Machines was exposed to continuous noise leves at 125.6% of the permissible daily exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level ot approximately 93.7 dBA during the 362 minute sampling period; exposure calculations include a zero inclrement for the 57 minutes for breaks.  The employer had not instituted a trianing program in accordance with the requirements of 29 CFR 1910.95(k) and ensured that employees participated in the training.  The employer shall ensure that each employee is informed of the following:  -The effects of noise on hearing;  -The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care; and  -The purpose of audiometric testing, and an explanation of the test procedures.  ABATEMENT CERTIFICATION REQUIRED
