29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Employees performing work in the yard including, but not limited to, customer service loading and unloading vehicles, and rebar fabrication, were exposed to struck-by hazards.  The employer did not evaluate the workplace to determine the types of hazards employees would be exposed to, such as fork truck and customer vehicle traffic, and evaluate ways to minimize exposure to the hazard.
