The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.   The employer failed to provide a forklift certification for employees that operate forklifts at 6544 L Street, Omaha, Nebraska 68117. 29 CFR  1910.184(c)(14):Employers must not use slings without affixed and legible identification markings.  Employees engaged in material handling operations located at 6544 L Street, Omaha, Nebraska 68117 were exposed to struck-by hazards in that employees were using slings that did not have identification markings to lift small motors in the maintenance shop.
