29 CFR 1926.417(b): Equipment or circuits that are deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized: a) Feeder (Building 9, 1st level): On or about June 2, 2014, an ineffective method was used to render the circuit inoperative and tags were not affixed to points where the circuit could be energized which exposed employees to electrical hazards up to electrocution. b) Power Distribution Unit (Building 9, 2nd level): On or about June 2, 2014, a method was not used to render the circuits inoperative and tags were not affixed to points where the circuits could be energized which exposed an employee disconnecting the power distribution unit to electrical hazards up to electrocution. c) Power Distribution Unit (Building 9, 2nd level): On or about June 2, 2014, a method was not used to render the circuits inoperative and tags were not affixed to points where the circuits could be energized which exposed an employee reconnecting the power distribution unit to electrical hazards up to electrocution.
