29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a) Transport Tech, LLC - On March 28, 2014, throughout the Central Transport, LLC, terminal, the floors in the dock and bay areas were not clean, as they contained wood chips, cardboard, and wood from broken pallets, and were not maintained in a dry condition, due to an accumulation of rain water from a leaking roof, lack of overhead doors, and the separation between the trailer. Employees were exposed to floors which were not maintained in a clean and dry condition, in the following instances: Instance #1: At the northwest forklift battery charging area, outside the dispatch office. Instance #2: North Bay Areas Instance #3: South Bay #55 Instance #4: South Bay #57 Instance #5-9: South Bay #59-63 Instance #10-11: South Bay #66-67 Instance #12: Near Bay #84-85 Instance #13: At Bay #53 b) Transport Tech, LLC - On June 11, 2014, throughout the Central Transport, LLC, terminal, the floors in the dock and bay areas were not clean, as they contained wood chips, cardboard, and wood from broken pallets, and were not maintained in a dry condition, due to an accumulation of rain water from a leaking roof, lack of overhead doors, and the separation between the trailer. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) On or about March 28, 2014 - The eyewash station located at the southeast battery charging station area at the Central Transport, LLC, terminal, was blocked by tires, did not contain sufficient water to provide adequate water flow, and was not accessible to employees for immediate emergency use. b) On or about March 28, 2014 - The eyewash station located at the northwest battery charging station area at the Central Transport, LLC, terminal, was blocked by equipment and pallets containing freight, and did not contain sufficient water to provide adequate water flow, and was not accessible to employees for immediate emergency use. c) On or about June 9, 2014 - The eyewash station located in the mechanic shop was not inspected and maintained, and was not accessible to employees for immediate emergency use. Transport Tech, LLC, was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.151(c), which was contained in OSHA Inspection Number 109333, Citation Number 1, item number 4, and was affirmed as a final order on January 6, 2012, with respect to a workplace located at 200 North Mannheim Road, Hillside, IL.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Transport Tech, LLC - The employer did not provide effective information and training to employees exposed to hazardous chemicals in their work area at the time of their initial assignment and whenever a new physical hazard was introduced into the work area. Chemical-specific information was not available through labels and material safety data sheets. Employees were exposed to chemicals including; Hawker batteries - containing 60% lead and 10-30% sulfuric acid; Johnson Controls batteries - containing 34% lead and 35 % sulfuric acid; PB Penetrating Catalyst Blaster - containing 40-50% naphtha solvent, 20-30% heavy petroleum distillate; Air1 Diesel Exhaust Fluid - containing 35% urea; Safety Kleen - Ice Breaker N1000 - containing 10-30% methyl alcohol, and propylene glycol; Zeco - Windshield Washer -20F - containing 31% methanol; Security Extended Life Anti-Freeze/Coolant Concentrate - containing 90-95% ethylene glycol, and sodium hydroxide; Haws Eyewash Additive Concentrate - containing 1% Chlorhexidine Gluconate and 10% Propylene Glycol. b) Transport Tech, LLC - The employer did not provide effective information and training to employees exposed to hazardous chemicals in their work area at the time of their initial assignment and whenever a new physical hazard was introduced into the work area. Chemical-specific information was not available through labels and material safety data sheets. Employees did not receive training by December 1, 2013, as required by the revised 2012 hazard communication standard, that included an explanation of the labels received on shipped containers and the workplace labeling system used by their employer, and the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information.
